Add an Insured
Welcome to the self service area of Policyholder Services, where you can add an insured risk to your policy. Coverage is not active until the application has been approved, your policy Declarations have been modified or endorsed, and the applicable premium (if any) as been received. For live assistance, we encourage you to speak with your underwriter. Contact information for your representative can be found here.
It is permissible for the employer or practice administrator to assist with the application process. However, the applicant (or an authorized representative of an entity applicant) applying as an additional insured risk must personally review and electronically sign the application. (For answers to common questions related to adding an insured, see Q&A section below)
When you click on a link below, you will be taken directly to the online application.
Who can submit the application
- If you are the authorized representative of an entity applicant, you may answer all questions, complete the acceptance page, and submit the application.
- If you are not the applicant nor an authorized representative of an entity applicant, you may answer some of the questions on the applicant’s behalf or leave the application blank before sending it to the applicant for completion and submission.
Forward a link
- To forward a link for the application to the applicant,
- Click “close” at the bottom of the screen and save your changes. You will be taken to a worklist page that displays pending applications (Work in Progress).
- Click “View” to access the selected application packet.
- On the next page, under “My Application,” click the “Notify” button.
- This will open a new window where you can enter the email address and name of the recipient.
- An email will be sent to the specified recipient with the necessary login information and instructions.
