Locum Tenens Application

A locum tenens is an ophthalmologist hired on a temporary basis to furnish direct patient treatment on behalf of, and only in the absence of, the Insured ophthalmologist. If the physician will provide services on a long-term basis (i.e., longer than 90 days) or will work concurrently with the employing ophthalmologist, please complete a Group Physician application instead.

Due to state patient compensation/excess liability fund prohibitions on the sharing of liability limits, locum tenens coverage is not available in Kansas, Nebraska, Pennsylvania, or Wisconsin.
