Will I be charged extra for refractive surgery coverage?

Coverage for extraocular refractive surgery procedures (such as PRK and LASIK) is automatically included within the standard Surgery Class 3 rating classification. Coverage for intraocular refractive surgery procedures (including refractive lens exchange and phakic implants) may be added by endorsement following underwriting review and approval of a supplemental questionnaire. Although the policy must be endorsed to include coverage for intraocular refractive procedures, no additional premium is charged for such endorsement.
