Nurse Anesthetist Application

If approved, coverage will apply solely to professional services rendered within the scope of the CRNA’s training, licensure, and employment by the insured ophthalmologist or professional entity. The nurse anesthetist must maintain separate coverage for any professional services rendered outside of this employment.

Due to state patient compensation fund requirements, coverage is not available to CRNAs who practice in Nebraska or Wisconsin.
