How can I get evidence of my OMIC coverage for my hospitals?

Include on your application any organizations, and their complete addresses, for which you need evidence of coverage. Should you need proof of coverage at any other time, you can submit your request by registering and accessing your account online. Or, you may fax, email, call, or mail us the necessary information. We will send the requested certificate of insurance to your hospital with a copy to you.
