Are refractive surgery procedures covered?

Yes.  Coverage for extraocular refractive surgery procedures (such as PRK and LASIK) is automatically included within the Surgery Class 3 rating classification. Although the standard policy excludes coverage for intraocular refractive surgery procedures, your policy can be endorsed to include coverage for refractive lens exchange (RLE) and/or phakic implants following review and approval of a supplemental refractive surgery questionnaire. If approved, coverage for the specified intraocular refractive surgery procedure will apply at full policy limits and without additional charge.
